The defibrillator and the ventilator are listed in Annex 1 of the Medical Device Operator Ordinance (MPBetreibV) and require a safety inspection at least every two years. The blood pressure monitor falls under Annex 2 and requires a metrological inspection. For devices in vehicles, there is a rule that many overlook: Where vibration, cold, and heat previously led to defects, Section 12 Paragraph 1 Sentence 3 of the MPBetreibV obligates the operator to..., before to have the two-year period reviewed.

Which device requires which test?

The type of inspection depends on the device, not the vehicle. Annex 1 of the Medical Device Operator Ordinance (MPBetreibV) lists the products subject to mandatory safety testing (STK), and Annex 2 lists those subject to mandatory medical device testing (MTK). A single device can fall under both categories.

Equipment in the rescue vehicle

Test

deadline

basis

Defibrillator, also known as a monitor defibrillator

STK

2 years, down to the month

Annex 1 No. 1.1, Section 12

ventilator

STK

2 years, down to the month

Annex 1 No. 1.5, § 12

Syringe and infusion pump

STK

2 years, down to the month

Annex 1 No. 1.4, § 12

Infant incubator, also known as transport incubator

STK

2 years, down to the month

Annex 1 No. 2, Section 12

Non-invasive blood pressure monitor

MTK

2 years, to the exact year

Annex 2 No. 1.3, § 15

Ear thermometer, forehead thermometer

MTK

1 year, to the exact year

Annex 2 No. 1.2.3, § 15

Electronic fever thermometer with probe

MTK

2 years, to the exact year

Annex 2 No. 1.2.1, § 15

Pulse oximeter, suction pump, charger

electrical test

after risk assessment

DGUV Regulation 3 or 4

The two deadlines run at different times, and this is the most common calculation error. The STK ends with End of month, the MTK with End of the year. A defibrillator tested in March 2025 is due for its next inspection at the end of March 2027. A blood pressure monitor from the same March has until December 31, 2027.

The rule for devices in the vehicle

Section 12, paragraph 1, sentence 3 of the Medical Device Operator Ordinance (MPBetreibV) requires earlier inspections if defects are to be expected sooner due to the specific usage and environmental conditions. The legislator provides no examples, but the conditions in the rescue vehicle are different from those in the treatment room: constant vibration, temperatures ranging from freezing to a heated vehicle compartment, humidity, frequent repositioning and plugging in.

Cables and connectors become loose earlier, visible as intermittent contact on the ECG trunk cable.

Batteries lose capacity faster when exposed to cold-warm temperatures than when stored indoors.

Cuffed hoses become porous where they run over an edge.

Sensor cables break at the strain relief because they are pulled during use.

Anyone who pushes the two-year inspection limit to the limit and overlooks a loose connection is not complying with the regulation. The decision regarding the inspection frequency belongs in the device documentation and is based on the usage profile.

What will change for layperson defibrillators on January 1, 2027

For automated external defibrillators intended for laypersons, the safety check (STK) can currently be waived: The device must be self-testing, and the operator must monitor it regularly. Section 12, paragraph 2 of the Medical Device Operator Ordinance (MPBetreibV) adds for newly put into operation Devices will have two additional requirements starting from January 1, 2027.

until 31.12.2026

From 01.01.2027, new devices

self-test

The device tests itself

The device tests itself

Results

remain in the device

documented and transmitted to the operator via remote monitoring

Visual inspection

regularly

event-related, depending on remote monitoring

Location data

no requirement

publicly available via the manufacturer

This exception applies only to devices intended for laypersons. The monitor defibrillator in the ambulance remains subject to mandatory safety testing (STK), regardless of its own testing capabilities. Anyone purchasing a publicly accessible AED from 2027 onwards must check for remote monitoring functionality before buying: A device without this function will no longer meet the requirements for exemption from the STK.

Who is responsible

The Medical Device Operator Ordinance (MPBetreibV) obligates the operator. This is the aid organization, the district, or the company that maintains the rescue equipment, not the testing service provider or the manufacturer. We conduct the inspection, document it, and affix the inspection sticker; keeping track of the deadline remains your responsibility.

Does a defibrillator in an ambulance require a safety inspection?

Yes. Defibrillators are listed in Annex 1, Section 1.1 of the Medical Device Operator Ordinance (MPBetreibV) and therefore fall under Section 12. The maximum period is two years, calculated from the end of the month of commissioning or the last inspection. The exception for self-testing devices applies only to automated external defibrillators (AEDs) intended for laypersons.

Are shorter inspection intervals used in emergency medical services than in hospitals?

The regulation does not specify a particular deadline for the emergency medical service. However, Section 12, Paragraph 1, Sentence 3 requires earlier inspections if, due to specific usage and environmental conditions, defects are to be expected sooner. Vibration, temperature fluctuations, and frequent unplugging and replugging within the vehicle are examples of such conditions. The two-year period is an upper limit, not a legal entitlement.

What will change regarding layperson defibrillators in 2027?

For devices newly commissioned from January 1, 2027, self-testing alone is no longer sufficient. They must document the results and automatically transmit them to the operator via remote monitoring. The operator must make information on the device's location, functionality, and public accessibility publicly available through the manufacturer. Existing devices are not affected.

How often must the ear thermometer in the rescue vehicle be checked?

Every year. Ear and forehead thermometers use infrared technology and are listed in Annex 2 under number 1.2.3, which specifies a one-year expiry date. Electronic thermometers with probes have a two-year expiry date under number 1.2.1. Both look identical on the shelf, and this is precisely where the correct identification often fails.

Does the DGUV inspection count as a safety inspection?

No, the two cover different things. The inspection according to DGUV Regulation 3 or 4 concerns electrical safety as work equipment, while the STK (Safety Test for Medical Devices) according to Section 12 of the Medical Device Operator Ordinance (MPBetreibV) concerns the medical device in its intended purpose. A device can require both. Which of the two DGUV regulations applies depends on the accident insurance provider: Regulation 3 applies to the employers' liability insurance associations, and Regulation 4 applies to the accident insurance funds of the public sector.

Can we have multiple vehicles inspected at once?

Yes. For larger quantities of equipment, we will schedule an appointment and conduct an on-site inspection to ensure that the rescue equipment is not tied up longer than necessary. Individual pieces of equipment can also be sent in without an appointment.

What we bring with us

Quality management system for metrological controls of medical thermometers, certified according to DIN EN ISO 13485

Reference measuring instrument with DAkkS calibration, regularly proven to the authority

MTK activity registered with the regional government and monitored from there

Over 18 years of testing experience with blood pressure monitors, over 10 years with medical thermometers

Small parts warehouse for repairs using original spare parts if the inspection reveals a defect

The prices for the individual exams are listed on Maintenance and repair.
